Tuition Advance/Reimbursement Request Forms

If you are a full-time employee, you may be entitled to a tuition advance or a tuition reimbursement.  Please contact Human Resources for further information.   We have provided the forms (PDF format) below for your use.  If you are eligible, please complete the appropriate form along with an expense report and submit the necessary documentation to Susan Dranski, Human Resources.
